This is the original form that I decided to re-design
I first decided that this form contained too much information to be effective for a single page and began working on a double sided design. I separated the information into two categories based on the projected
user experiences of both patient and care giver.
I then chose a font that would compliment to tone of the business
and would look more professional to the patient.
